Day-to-day and acute care is solid at Lecce's regional hospital, Ospedale Vito Fazzi (listed in the US Embassy's medical directory). But for genuinely world-class/complex care you travel: Bari's university hospitals are ~90 min by train, and the top national centers are in Rome/Milan. English-speaking doctors are scarce outside private clinics — you'll often want an Italian-speaking helper or a bilingual private GP.

Spring and autumn (16–21°C/60s–70s°F) are the sweet spot. Not warm year-round, but no real cold.
Italy sits at US State Dept Level 2 ("Exercise Increased Caution," raised from Level 1 in 2025) — a blanket EU-wide terrorism/civil-unrest posture shared by France, Germany, UK, not an Italy-specific warning. Violent crime in Lecce/Salento is low; the realistic risks are petty theft and pickpocketing in tourist zones and occasional non-violent property crime. Comfortable for a 55–70 couple, including walking at night in the centro storico.
Real but modest and Italian-integrated — nothing like the turnkey Anglophone scenes of the Algarve or Costa del Sol. A growing trickle of American/Northern-European retirees (partly drawn by the 7% tax towns), some Facebook groups and Salento expat meetups, but you will be the foreigner who learns Italian, not one of a crowd. Best for someone who wants immersion, not an expat bubble.
No direct US flights. Nearest airport is Brindisi-Salento (BDS), ~40 min away, well-connected to Rome and Milan; from there it's a transatlantic hop. Realistic door-to-door from the US East Coast is ~13–16 hrs with one (often two) stops. Fine for an annual visit home; not somewhere you pop back to the States on a whim.
Italy taxes residents on WORLDWIDE income. Standout incentive: 7% flat tax on all foreign-source income for 10 years for new-resident foreign pensioners — but requires 5+ years prior non-residence AND registering in a municipality under 30,000 people (threshold raised from 20,000 in Apr 2026), so Lecce city (~95k) is excluded; nearby Salento towns qualify. US–Italy tax treaty in force; US persons still file with the IRS and use foreign tax credits.
