The stated 2026 minimum stable PASSIVE income is ~€31,000/yr single (~€2,580/mo; shown ~$2,950/mo) and ~€38,000/yr for a couple (+~20% spouse, +5%/dependent), from pensions/Social Security/investments/rentals — savings alone don't count and a strict no-work clause applies. Apply at your home US consulate before moving, hold a 12-month lease or owned property in your name, then file for a permesso di soggiorno within 8 working days of arrival. Pitfalls: consulates have wide discretion and frequently demand income above the floor (advisors often target ~€100k/yr), reject thin/short-term housing or non-passive income, and processing runs up to ~3 months. Renewable, with a path toward long-term residency/citizenship.

Healthcare. Adequate for routine and most acute needs, but Sardinia's public system is visibly under strain — an honest caveat. The Azienda Ospedaliera Brotzu is the island's largest hospital and main referral center (cardiology, transplant, emergency), and the AOU di Cagliari university hospital adds teaching depth. However, 2025-26 Sardinian press (L'Unione Sarda) repeatedly reported ER overcrowding, ambulance backlogs at Brotzu and staffing/political turmoil — so wait times and capacity are a real concern. English isn't guaranteed publicly; expats lean on private clinics and insurance. As an island, the nearest mainland world-class centers (Rome ~1h flight, Milan) are the fallback for complex cases.
Climate. Hot-summer Mediterranean and one of Italy's sunniest spots (~255 sun hours/month). Summers are hot and dry (Jul-Aug highs ~30-31C/86-88F, low-ish humidity for the coast); winters are mild but cool and breezy (Jan highs ~14C/57F, lows ~7-8C/45F), rarely near freezing. Defining feature: WIND — the maestrale can blow hard for days, bracing in winter and a real daily factor. Rain is modest (~5-6 rainy days/month, autumn/winter peaks); summers are bone-dry.
Safety. One of the safest provincial capitals in Italy. Reported crime runs roughly 16% below the national average; violent crime against residents or visitors is rare, and Sardinians widely describe walking around town at night as comfortable. The realistic risks are ordinary petty theft and pickpocketing in crowded/tourist spots and on transit, plus seasonal beach-bag opportunism — secure valuables and use normal urban awareness. The compact old town (Castello) and the Marina/Villanova districts are easy at most hours. Solo women generally report Cagliari as relaxed and low-hassle by Italian-city standards.
US taxes abroad. Italy taxes residents on WORLDWIDE income at progressive 23-43% rates plus surcharges.
