The public Policlinico di Bari (with the Giovanni XXIII pediatric hospital) is one of southern Italy's largest — 1,500+ beds, the University of Bari medical faculty, and European Reference Network status across transplantation, oncology, cardiology and rare disease; Ospedale San Paolo and Di Venere cover wider acute needs. Public SSN care is low-cost and broadly capable but, as across the south, slower than the north, with English not guaranteed — so most expats add private insurance and use private specialists for speed. For the most complex elective cases, mainland hubs (Rome ~1h flight, Milan) remain the fallback.

Summers are hot and sunny (Jul-Aug highs ~28-30C/82-86F, nights ~20-22C, ~56% humidity, sea breezes help); winters are mild but cool and can be windy (Jan highs ~11-14C/52-57F, lows ~5-7C/41-45F), essentially no snow. Rain is modest (~565mm/yr, Nov peak; June-July nearly dry). Spring and autumn are the sweet spots; not warm year-round, but no real cold.
Comfortable for a 55-70 couple in the central and seafront areas, with violent crime against visitors statistically negligible. The realistic risk is petty crime: pickpocketing, bag-snatching and opportunistic theft in crowds, on transit and around the station; regional police data show Bari's petty-theft rate trending down since 2022. The Murat grid (elegant 19th-century center), the Lungomare seafront and Bari Vecchia by day are well-lit and busy; Bari Vecchia warrants more awareness after ~11pm, and peripheral San Paolo, San Pio-Enziteto and Fesca are higher-crime and best avoided at night. Standard urban caution suffices for solo women in the central zones.
Small and Italian-integrated — much less of an expat scene than the Salento/Itria Valley towns that draw retirees to Puglia. As a working port and university city, Bari has some international student and business presence, a few Facebook groups and English-speaking services, but daily life, landlords and bureaucracy run in Italian. Expect to be the foreigner who learns the language; best for someone wanting authentic immersion plus an airport with a US nonstop.
Puglia's quiet ace: United now flies a NONSTOP Newark (EWR)-Bari (BRI) route (~10h10m), launched in 2026 as the first North American service to Puglia — a rarity for the deep south, though seasonal-leaning, so confirm dates. Otherwise BRI's only intercontinental links are Africa/Middle East; off-peak US trips connect via a European hub or Rome/Milan, ~12-15h door-to-door. With the nonstop, Bari beats Bologna or Cagliari for US access.
Italy taxes residents on WORLDWIDE income at progressive 23-43% rates plus surcharges. Puglia IS one of the eight southern regions eligible for Italy's 7% flat tax for foreign pensioners — but ONLY in municipalities of 30,000 or fewer residents (raised from 20,000 on 7 Apr 2026, unlocking 18 Puglia towns), so Bari (~320k) does NOT qualify; many smaller Puglia/Salento towns do if you register there. The US-Italy treaty prevents double taxation (US Social Security generally taxable only in Italy), but US citizens still file with the IRS plus FBAR/FATCA.
