Routine care runs through Tavira's public health centre (Centro de Saúde); the Algarve's main public hospital, Hospital de Faro, is about 35 km (~40 min) away, alongside well-regarded private options — the Hospital Particular do Algarve group and Lusíadas in the Faro area. The region's large expat community means English-speaking GPs and dentists are easy to find, and private care costs a fraction of US prices. D7 residents can use the public SNS; most everyday and acute needs are handled locally, but complex or specialised treatment usually means a trip to Lisbon (~2.5–3 h).

Eastern Algarve is calmer and slightly warmer than the west.
US State Dept Level 1 (exercise normal precautions), as of Mar 2026 — Portugal is among Europe's safest countries. Numbeo safety index ~71 (Jul 2025, small 15-person sample). Violent crime is rare; main risk is petty theft/pickpocketing in tourist-heavy spots and summer crowds. One outlier Numbeo comment alleges a robbery, but it runs against the overwhelming consensus of a very safe town.
Established, older, mostly-retiree international community — predominantly British, plus French, German and Dutch; smaller and more integrated than the western-Algarve expat hubs (Lagos, Albufeira). Tavira stays distinctly Portuguese: big enough for English-speaking services and social circles, small enough that you'll actually live among locals.
Nearest airport is Faro (FAO), ~30 km / 30-40 min. United runs a seasonal nonstop Newark-Faro (~7h20, ~4x/week, summer only as of 2026); year-round you connect through Lisbon (45-min hop from Faro) onward to JFK/EWR/BOS/MIA/IAD on TAP, Delta, United or JetBlue. Workable but typically a one-stop, full-day trip outside summer.
The key downside. Portugal taxes residents on WORLDWIDE income at progressive rates (~13%-48%). The NHR regime that exempted foreign pensions ended (closed to new entrants 2024, transition ended Mar 31 2025); its replacement, IFICI/"NHR 2.0," is for qualified innovation/R&D professionals and does NOT cover retirees — so a US retiree relocating now faces full Portuguese tax on foreign pensions and is taxed on Social Security as a resident. A US-Portugal tax treaty and a Totalization Agreement exist (foreign tax credits prevent true double taxation, and you keep collecting US SS), but Americans still file US returns due to citizenship-based taxation. Get a cross-border advisor before committing.
