Day-to-day care is strong and cheap: Hospital São Gonçalo in Lagos handles primary/minor needs, and a dense private network (Hospital Particular do Algarve in Gambelas/Alvor, British Hospital Algarve) offers short waits and reliably English-speaking doctors; private insurance ~€50-100/mo. Acute/emergency care funnels to the public Hospital de Faro (~1hr10 by car), the region's only major public hospital and known to be busy/strained. Truly world-class tertiary care (complex cancer, cardiac, neuro) is in Lisbon — ~2.5-3hr drive or short flight. Good, not elite, for a serious diagnosis.

Rain concentrates Nov-Feb. Winters are mild but not tropical-warm, and many homes have poor heating/insulation, so it can feel cold indoors.
US State Dept Travel Advisory Level 1 (Exercise Normal Precautions) for Portugal as of 2026 — the safest tier. Violent crime against foreigners is rare. Numbeo's Lagos Safety Index sits around 61 (moderate-good; thin data, ~21 contributors, Dec 2024). The real issue is petty/property crime — pickpocketing, theft from cars and rentals — which rises sharply in the summer tourist crush.
The Algarve hosts one of Europe's largest, most established English-speaking expat/retiree communities (roughly 100,000 foreigners region-wide, far above Portugal's national average). Lagos specifically draws a slightly younger, more active crowd, with English widely usable in shops, restaurants, healthcare and services — an unusually soft landing for Americans.
Limited direct US access: United flies Newark (EWR)-Faro seasonally (~4x/week, ~7h20, from mid-May 2026) — the only nonstop. Every other US city routes through Lisbon (TAP/Delta/JetBlue/United from JFK, Boston, Miami, DC, Newark), then ~2.5-3hr overland to Lagos or a short hop to Faro (~1hr10 drive away). Workable but usually a one-stop, long-ish travel day.
Worldwide taxation for tax residents (183+ days/yr). The NHR flat-tax regime closed in 2024 (legacy holders keep ~10-yr benefits); its replacement IFICI does NOT cover pensions. For new retiree-residents: US private pensions and 401(k)/IRA distributions are taxable in Portugal with the US foreign tax credit offsetting US tax; US Social Security and US government pensions are treaty-protected and taxed only in the US. A US-Portugal income tax treaty and a Social Security totalization agreement both exist.
